E. J. Smith, captain of the Titanic, received  at least 5 separate warning transmission about icebergs on June 14th. The set of events that led up to the sinking remains relevant to leaders and entrepreneurs even today. What happened to the 5 warnings?

When captain Smith recieved his first warning on the morning of that fateful day, he passed the message on to the bridge with no instruction and did nothing else. The second warning, recieved early afternoon was showed to the Chairman of the White Star Line, again without instruction from the captain, who changed course marginally, but didn't alter speed. His priority was setting a record time for a luxury liner to make the crossing.

A couple of hours later, the Chairman posted the second warning to the bridge without instruction just before Smith attended a dinner party celebrating his retiring voyage, during which a third warning was recieved and overheard by the crew. Smith did leave early to talk with his second officer but then went immediately to bed.

The last two warning tranmissions were cut off by the crew due to an overload of messages from passengers.

With binocular's locked up and inaccessible by the crew and the ship going at a fast pace, even when the iceberg was spotted, any effort to change course was in vain and the ship made the fatal collision.

These same mistakes appear in many forms when running a business. Ignoring warnings, vague instructions and neglecting the tools at your disposal can all happen without a focus on "business navigation".
